Admissions

Admission day can feel very stressful for residents as well as family. We understand that major health events, hospital stays and declining health are not easy to navigate. We strive to make the admissions process as seamless and comfortable as possible.

Our compassionate admissions team is here to help with all your questions and provide you with guidance .

In order to be considered for admission, you must have the following:

• Order from primary care physician, hospital etc. to admit for a skilled need.
• History and Physical medical information performed within the last 30 days.
• Chest X-Ray or TB test to rule out Tuberculosis in last 6 months.
• Therapy notes (if applicable) 
• Current medication list completed by physician.

It is helpful to bring the following information upon admission to ensure a smooth transition:

• Medicare and/or Medicaid Card
• HMO or gap filler insurance card
• Living Will and/or Power of Attorney
• Emergency contact information
• Picture ID
